Buttock augmentation with autologous fat? Questions for PD Dr Mathias Tremp

In recent years, buttock augmentation with autologous fat has become increasingly popular. Fat is suctioned from the abdomen, for example, and after it has been cleaned and prepared, it is inserted into the buttock area. An alternative option is buttock augmentation using implants.

When is buttock augmentation with autologous fat suitable?

This technique is suitable if there is enough fat and at the same time a shape improvement is desired, for example on the abdomen or thighs. Basically, the fat can be taken from different regions and slim patients are also suitable for this technique.

How deep is the fat inserted?

If the fat is injected into the muscle, there is a risk of so-called fat embolism (vascular occlusion by the injected fat). To avoid this, we inject the fat superficially into the fatty tissue. The safety of the fat injection can be increased by ultrasound.

When is buttock augmentation with implants suitable?

For very slim patients who want a larger and shapely bottom, buttock augmentation with implants is recommended. The implants are placed in the muscle.

Which is the better method for buttock augmentation: autologous fat or implants?

If there is insufficient fat and a significant increase in volume is desired, then buttock augmentation with implants is the better method.

What are the alternatives to autologous fat treatment rsp. buttock augmentation with implants?

Collagen stimulators are an alternative option for buttock augmentation and lifting. Collagen stimulators are high-quality dermal fillers consisting of calcium hydroxyapatite or based on poly-L-lactic acid (PLLA). The products are "naturally" broken down by the body. At the same time, the body's own collagen and elastin production is reactivated. Treatment with hyaluronic acid would also be a possible alternative to buttock augmentation. This so-called biostimulation leads to an increase in volume, firming and cellulite can be optimized.

What do I have to consider before the operation?

A nicotine stop 3 weeks before surgery is mandatory. In addition, we recommend a nutritional optimization (protein-rich diet, omega-3 fatty acid, probiotics, Arnica globules) 3 weeks before the operation to improve wound healing. You will receive a disinfecting solution from us, which you should use the day before. You should also start manual lymphatic drainage the day before to optimally prepare the tissue for the operation.

Does the buttock augmentation with autologous fat have to be performed several times?

A certain percentage (about 40-50%) of fat grafting dissolves again in the first 3-6 months after surgery. Depending on the desired volume increase, a second treatment may need to be performed again.

Can a buttock lift be performed at the same time as the buttock augmentation?

This combined treatment is of course possible. Buttock lift is especially suitable for patients who have lost a lot of weight. However, the scars must be taken into account. Buttock augmentation with implants or autologous fat alone only results in a minor buttock lift.

Where is the scar for buttock augmentation ?

In the case of buttock augmentation with implants, the scar is located directly in the buttock cleft and is approx. 4 - 5 cm long. In the case of buttock augmentation with autologous fat, the scars are approx. 3 - 4 mm long and are hardly visible after one year.

What kind of anaesthesia is necessary for buttock augmentation?

Buttock augmentation with autologous fat or implants is performed under general anesthesia. Hospitalization is foreseen for 2 - 3 days

What do I have to keep in mind after the operation?

After the operation, you should exercise and make sure you drink enough fluids. Full bending in the hip is restricted for one week. You will be unable to work for approx. 3 - 5 days with autologous fat and approx. 10 - 14 days with implants. You should sit as little as possible for 14 days. To minimize the risk of thrombosis or infection, we will give you a thrombosis or antibiotic prophylaxis for 10 - 15 days. In the first 4 days, the buttocks must be relieved in the ring cushion.

Showering is permitted from day 1 with the appropriate disinfectant. From the 5th day after the operation, you should sleep on your stomach for one month. A high-protein diet should be followed for one month after the operation. Stopping smoking for 3 months after the operation is mandatory. In addition, manual lymphatic drainage (15 treatments) is recommended from the 5th day after the operation. You will be given a compression suit to wear for 6 - 8 weeks. During this time, the buttocks should also be relieved with a special cushion (BBL cushion).

What are the risks of buttock augmentation?

Swelling, infection or bruising may occur. Because the fat is injected in a superficial layer, the risk of vascular occlusion is very small. After the autologous fat treatment, there is a possibility that a relevant percentage (40-50%) may dissolve again. Then a second treatment may be necessary or the use of implants should be discussed initially.

What are the costs of buttock augmentation?

The procedure is mainly aesthetic and is very rarely covered by health insurance. The costs depend on the type of procedure and are adjusted individually.
